Job summary

Qualifications

• High school diploma or equivalent required; college education preferred.
• Experience or knowledge of SSI/SSDI programs.
• Medical terminology preferred.
• Prior medical office or hospital experience preferred.
• Basic Microsoft Office skills required.
• Experience with HHSC or DADS a plus.

Responsibilities

• Identifies patient and family needs and connects to hospital, community, state, and federal resources.
• Coordinates transportation, lodging, meals, and basic needs related to discharge planning.
• Serves as liaison between patients, families, clinical staff, departments, insurers, and community agencies.
• Assists with discharge planning and documentation to support safe, timely discharge.
• Maintains knowledge of community resources and records referrals and follow-ups.
• Answers departmental calls and routes inquiries, escalating concerns as needed.
