Driscoll Children's Hospital in Brownsville, TX is seeking a Service Coordinator III to advance person-centered care coordination for STAR Kids members. You will support the assigned RN/SW team, staff the care coordination pod, and facilitate timely access to community-based LTSS while maintaining confidentiality and regulatory compliance.

The role requires knowledge of Texas Medicaid criteria, managed care contracts, and collaboration with physicians, families, and care teams to optimize

Qualificações

• Experience as a case management support technician or LTSS coordinator preferred.
• High school diploma or equivalent required.
• At least an Associate's Degree preferred.
• Experience in a managed care environment preferred.
• Knowledge of Texas STAR/KIDS programs preferred.

Responsabilidades

• Provide administrative support for care coordination.
• Provide outreach and care coordination to Level 3 members.
• Coordinate care and utilization of health care resources for STAR Kids patients.
• Assist members in identifying, selecting, obtaining, coordinating, and using services to enhance wellbeing and independence.
• Answer Service Coordination Pod calls.
• Produce status reports tracking STAR Kids members.
• Ensure knowledge of STAR Kids contract and requirements.
• Participate in performance improvement activities.
• Accept other assignments as requested.
