Job summary

Driscoll Health System is seeking a Service Coordinator III to advance person-centered care coordination for STAR Kids members. You will collaborate with RNs, social workers, and physicians to ensure timely LTSS services and reduce institutionalization risks.

The role emphasizes contractual compliance, multidisciplinary communication, and efficient care utilization across the care team, with a focus on children and families in the South Texas region.

Qualifications

• Experience in case management support or LTSS coordination in health plans preferred.
• High school diploma or equivalent required.
• At least a 2-year college Associate’s Degree preferred.
• Previous managed care experience preferred.
• Direct experience with children and young adults with similar conditions preferred for Level 3 contacts.
• A 4-year college degree and/or LVN license preferred.

Responsibilities

• Provide administrative support for care coordination to the assigned RN and SW staff.
• Provide outreach and care coordination to Level 3 members as warranted.
• Coordinate care and efficient utilization of health care resources for STAR Kids participants.
• Assist members in identifying and coordinating services to enhance wellbeing and independence.
• Refer members to disease management and respond to incoming calls for the Service Coordination Pod.
• Maintain status updates and ensure compliance with STAR Kids contract requirements.
