Responsibilities

• Provide comprehensive psychosocial assessment, evaluation, treatment planning, education, and discharge planning to support identified or potential barriers to care.
• Assist Nursing, Physicians, and Case Managers in discharge planning and coordination of care; work with outside agencies for continuity of care.
• Coordinate and participate in discharge planning activities, information and referral to community resources, and follow-up care needs.
• Liaise with local Social Work schools, onboard new students, and support training and supervision of staff/students.
• Provide supportive counseling and crisis intervention for patients and families as part of psychosocial services.
