Job summary

Child and Family Services, Inc. is seeking a Senior Success Coach to provide intensive case management for youth and families in New Bedford public middle schools. The role is full-time (40 hours/week) and embedded in schools, reporting to the ASPIRE Program Director with a support team.

Responsibilities include intake, needs assessment, service planning, advocacy, psychoeducation, and collaboration with families, school staff, and community partners to coordinate resources and monitor outcomes.

Qualifications

• Master’s level degree in Counseling, Social Work, Psychology, or a related field.
• Experience working directly with youth, families, and substance use issues.
• Ability to work with culturally diverse populations.

Responsibilities

• Provide a safe, welcoming, and clean environment for clients and families.
• Collaborate with team members, clients, and families to implement person-centered and family-centered care.
• Conduct intake interviews, needs assessments, and develop service plans.
• Provide psychoeducation and advocacy as appropriate.
• Hold case management meetings with parents/caregivers to discuss progress and update plans.
• Maintain thorough documentation of meetings, releases, referrals, and communications.
• Advocate for youth and families and design individualized youth development strategies.
• Develop and maintain long-term relationships with families and identify community resources.
