Job summary

Qualifications

• Bachelor's Degree in Nursing required; Master's Degree in Nursing preferred.
• 5-7 years of inpatient case management experience.
• 5-7 years of discharge planning experience.
• Massachusetts RN license required; ACM/CM or related certifications preferred.

Responsibilities

• Act as a case manager expert and mentor, resolving issues impacting care transitions.
• Co-manage complex patient discharges with unit-based case managers.
• Identify discharge barrier mitigation strategies during routine huddles and meetings.
• Share new escalation pathways and resources with the case management team.
• Execute and enforce best practices related to case management.
• Provide mentorship and guidance on complex care and post-acute discharge.
• Monitor ongoing performance initiatives and develop recommendations for improvement.
• Analyze trends in lengths of stay, readmissions, and utilization.
• Coordinate efforts to optimize hospital days and discharge planning.
• Stay current on evidence-based practices and healthcare regulations.
