• Monitor ongoing performance initiatives and develop recommendations for improvement.
• Analyze trends in lengths of stay, readmissions, and utilization.

Licenses & Certifications:

• Current RN registration in Massachusetts
• Accredited Case Manager - Registered Nurse [ACM‑RN] preferred
• Accredited Case Manager [ACM] preferred
• Case Management Administrator-Certified [CMAC] preferred
• Nursing Case Management [CMGT-BC] preferred
