Job summary

Mass General Brigham in Boston is seeking an experienced Registered Nurse Case Manager to join the Case Management department. You will assess and plan care, coordinate services, and support discharge planning to ensure high-quality patient care.

This position requires MA RN license, an associate degree or bachelor’s in nursing, and 2–3 years of case management or discharge planning experience. Onsite work at 265 Charles Street with a 30-hour weekly schedule.

Qualifications

• Associate's Degree in Nursing or Bachelor's Degree in Nursing preferred
• MA RN License required
• 2-3 years experience in case management, utilization review, or discharge planning preferred
• Proficient in Microsoft Office and related software
• Ability to establish strong rapport with patients and staff
• Knowledge of Healthcare and Managed Care preferred

Responsibilities

• Client assessments and care planning.
• Modify patient treatment plans based on responses and conditions.
• Prepare patient/family for discharge.
• Communicate with third-party payers to obtain authorization for reimbursement.
• Review cases with medical directors as needed.
• Define care goals and provide education to patients and families.
• Maintain accurate reports and records.
• Collaborate with care teams to coordinate services.
