Job summary

MarinHealth seeks a skilled RN Case Manager to lead the development and implementation of interdisciplinary care plans for hospitalized patients. You will determine the appropriate level of care and supervise discharge planning, ensuring high-quality, cost-efficient outcomes while coordinating with physicians, staff, and hospital leadership.

The role requires a BSN, 3+ years in acute care (medical/surgical or critical care), and RN licensure with BLS.

Qualifications

• Bachelor of Science in Nursing preferred.
• Three (3) or more years of experience in an acute patient care setting, preferably in medical/surgical or critical care.
• Licenses/Certifications required: RN license, Basic Life Support (BLS).IAMTAC within 30 days of hire.

Responsibilities

• Lead the development and implementation of the interdisciplinary plan of care.
• Determine appropriate level of care and supervise discharge planning.
• Act as liaison between physician, staff, and hospital leadership for patient care."
• Coordinate care to ensure efficient patient throughput and quality outcomes.
• Provide guidance to the physician and escalate to Medical Director when necessary.
