A leading healthcare staffing firm is seeking an experienced Hospital Registered Nurse for a prestigious local hospital in San Rafael, California. This position focuses on case management, requiring an RN license in California and a minimum of one year of acute care case management within the last three years. Responsibilities include developing care plans, performing utilization reviews, and collaborating with healthcare teams. Qualifications

• Must have clear and active RN license in CA.
• BSN or equivalent education/experience in nursing or health related field.
• Acute care case management required within a hospital setting (minimum 1 year within the last 3 years) with references to reflect.
• Previous case management experience and demonstrated experience in utilization management, discharge planning, or transfer coordination.
• Knowledge of Nurse Practice Act, The Joint Commission and other federal/state/local regulations.

Responsabilités

• Develops plans of care and discharge plans in conjunction with physicians.
• Performs daily pre-admission, admission, and concurrent utilization reviews.
• Escalates utilization and system problems unresolved at the local level.
• Monitors the progression of the plan of care and facilitates discussions.
• Educates healthcare team members on utilization and cost containment initiatives.
• Collaborates with patients, families, physicians, and staff regarding care provisions.
• Counsels on the application and utilization of member health care benefits.
