Kindred Hospital San Francisco Bay Area, a 99-bed long-term acute care facility, seeks a Care Coordinator to drive patient care through the Interdisciplinary Care Transitions team and ensure optimal resource use and continuity of care.

The role supports discharge planning, utilization reviews, and collaboration with physicians, payors, and families to achieve high-quality, cost-effective outcomes. RN licensure required with preferred BSN or MSW/BSW licensure.

Qualifications

• Knowledge of government and non-government payor practices, regulations, standards and reimbursement.
• Knowledge of Medicare benefits and insurance processes and contracts.
• Knowledge of accreditation standards and compliance requirements.
• Must read, write and speak fluent English.
• Excellent interpersonal, verbal and written communication with leadership, physicians, payors and others.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team.
• Monitor patient stays for effective care coordination.
• Refer high risk patients for additional support.
• Collaborate with physicians, payors and families in planning care.
• Discharge planning and care transitions coordination.
