Job summary

Kindred Hospital San Francisco Bay Area is seeking a skilled RN Case Manager to coordinate patient care across the continuum. The role focuses on collaboration with ICT members, discharge planning, and ensuring cost-effective, high-quality care.

You will engage with interdisciplinary teams, manage resources, and advocate for patients and families to support informed decisions. Experienced RNs with case management exposure are preferred.

Qualifications

• Graduate of an accredited nursing program with RN licensure.
• BSN preferred; MSW/BSW licensure may be considered where applicable.
• Two years of healthcare experience preferred, with case management exposure.

Responsibilities

• Coordinate and facilitate care with Interdisciplinary Care Transitions team.
• Monitor patient stay for effective care coordination and resource use.
• Conduct discharge planning and comprehensive assessment.
• Engage in utilization review and authorization with payors.
• Collaborate with physicians, families, and post-acute providers.
• Participate in interdisciplinary rounds and plan of care development.
