Job summary

MarinHealth is seeking a Director of Care Coordination to lead strategic and operational efforts across the acute-care continuum. The role partners with physicians, Nursing, Social Work, and post-acute providers to ensure effective case management, discharge planning, and safe transitions of care.

Responsibilities include developing programs, managing budgets, ensuring regulatory readiness, and driving quality improvement using clinical, operational, and financial data.

Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Master’s degree in nursing, Health Administration, Business Administration, or similar healthcare field highly desired.

Responsibilities

• Provide strategic and operational leadership for care coordination, case management, utilization management, discharge planning, and psychosocial coordination.
• Establish department goals, monitor performance, and develop programs, policies, workflows, staffing models, and quality initiatives.
• Lead data-driven improvement, budget management, and regulatory readiness for MarinHealth’s care coordination functions.
• Collaborate with physicians, Nursing, Social Work, Revenue Cycle, Health Information Management, and post-acute partners to ensure optimal patient progression and safe transitions of care.
• Ensure staffing plans and resources align with organizational priorities and patient needs.
