Job summary

MarinHealth is seeking a Director of Care Coordination to lead case management, utilization management, and discharge planning across the acute-care continuum. The role partners with physicians, Nursing, Social Work, Revenue Cycle, and post-acute providers to improve patient progression, control costs, and ensure quality outcomes.

The ideal candidate has a BSN, 5+ years of leadership experience in acute care, and a California RN license.

Qualifications

• BSN required.
• Master’s degree in nursing/health admin/business admin or related field preferred.
• Minimum five years of leadership/administrative experience in case management or care coordination.
• California RN license required at hire; BLS required at hire; CCM/ ACM preferred.
• Strong budget, staffing, productivity, and data-driven improvement skills.

Responsibilities

• Provide strategic and operational leadership for care coordination and utilization management.
• Set department goals and manage budgets, staffing, and quality initiatives.
• Develop and evaluate care coordination processes across the hospital continuum.
• Ensure effective discharge planning and safe transitions of care.
• Lead performance improvement activities and ensure regulatory readiness.
