New Hanover County is seeking a dedicated Social Worker for the Senior Resource Center Life Enrichment Program. The role focuses on assessments, resource assistance, and home-based or county-facility support to help older adults maintain independence and quality of life.

Responsibilities include case monitoring, care planning, home visits, documentation, and coordinating services across agencies. Bilingual Spanish speakers receive a 5% language differential; full-time, non-exempt status; hours

Qualifications

• Knowledge of social work principles and community resources.
• Ability to prepare written reports and communicate clearly.
• Experience with abuse/neglect investigations and protective services.

Responsabilités

• Conduct ongoing case monitoring, assessments, and care plan evaluations.
• Perform home visits to monitor progress and identify risks.
• Maintain accurate documentation of client interactions and plans.
• Coordinate social services and referrals with community partners.
• Deliver Options Counseling and support informed long-term decisions.
• Engage in professional development and maintain required certifications.
• Advocate for clients and collaborate with providers and organizations.
• Handle multiple tasks in a fast-paced, service-focused environment.
• Ensure attendance and readiness for emergency/disaster work as needed.

Formation

MSW

BSW + 1 year related social work experience

Four-year degree + 15 semester hours in social work + 2 years related experience

Four-year degree + 3 years related social work experience
