New Hanover County seeks a dedicated Social Worker for the Senior Resource Center Life Enrichment Program (LEP) team. The role involves assessments, resource referrals, and in-home or facility-based support to older adults and caregivers to promote independence and quality of life.

Responsibilities include case monitoring, care planning, and coordination with community partners; bilingual English/Spanish skills are a plus as is experience with Options Counseling and grant-funded programs.

Qualifications

• MSW or BSW with relevant experience required.
• Bilingual in English and Spanish strongly preferred.
• Knowledge of social work principles, community resources, and ability to prepare written reports.

Responsabilités

• Conducts ongoing case monitoring and evaluation, including assessments and care plan development.
• Performs home visits to monitor progress and address concerns.
• Maintains accurate documentation of client interactions and service plans.
• Coordinates social services across agencies by referrals and collaboration with partners.
• Provides Options Counseling to support informed long-term service decisions.
• Engages in professional development through training and certifications.
• Advocates for clients by fostering relationships with providers and community organizations.
• Utilizes technology to manage multiple priorities in a fast-paced environment.
• Performs work with regular attendance and meets scheduling needs.
• Performs duties during emergency/disaster situations.

MSW from an accredited school

BSW with 1 year related experience

Four-year degree + 15 semester hours in social work + 2 years related experience
