Job summary

Uintah Basin Rehabilitation and Senior Villa in Roosevelt, Utah is seeking a Physical Therapist for a full-time position on site. You will evaluate residents, develop individualized treatment plans, and provide therapy to improve mobility and function.

Qualifications

• Current Physical Therapist license in the State of Utah.
• Graduate of an accredited Physical Therapy program.
• Strong clinical and assessment skills.
• Strong communication and interpersonal skills.
• Ability to work effectively within an interdisciplinary team.
• Ability to maintain accurate documentation and meet regulatory requirements.

Responsibilities

• Evaluate residents and participate in therapy screenings.
• Develop and implement individualized treatment plans.
• Provide physical therapy services to improve mobility and function.
• Collaborate with the interdisciplinary care team to achieve resident goals.
• Educate residents and families on treatment plans and progress.
• Maintain timely and accurate clinical documentation.
