Job summary

Vista Ridge Care and Rehab in Pueblo, CO is seeking a compassionate Physical Therapist to join our in-house therapy team serving long-term residents. The role involves creating personalized treatment plans, guiding patients through exercises, and coordinating with the rehabilitation staff to improve mobility and quality of life.

We offer a competitive pay rate of $45-$55 per hour, flexible scheduling including full-time, part-time, and PRN options, plus benefits and a supportive, team-oriented

Qualifications

• Compassionate and patient-focused demeanor
• Strong communication with patients and families
• Ability to work in a team-based rehab setting
• Familiarity with geriatric patient needs
• Understanding of evidence-based practice in PT

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Diagnose movement dysfunction by observing patients during movement.
• Develop individualized treatment plans.
• Coach patients through therapeutic exercises and modalities.
• Supervise and support physical therapy assistants and rehab aides.
• Maintain accurate patient records and document progress.
• Provide discharge summaries and counseling to patients and families.
• Participate in trainings, in-services, and conferences.
