Heritage Park Rehabilitation & Healthcare in Roy, UT is seeking a Full Time Physical Therapist to join our in-house rehab program. You will screen and evaluate patients, develop care plans, and deliver skilled treatments under regulatory standards.

Licensed PT is required; new grads are welcome. We offer a $5,000 sign-on bonus, comprehensive benefits, 401k, and a collaborative team focused on compassionate geriatric care.

Qualifications

• Licensed Physical Therapist required.
• Open to therapists at all experience levels; new grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care, creating care plans per standards.
• Deliver rehabilitative, skilled and medically necessary treatment interventions.
• Perform procedures including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Provide consultation to patients, families and caregivers regarding physical disorders.
• Generate discharge summaries ensuring regulatory compliance.
