Job summary

Help At Home, LLC is seeking a Qualified Developmental Disability Professional (QDDP/DDP) to support individuals in Brunswick, GA. The role focuses on person-centered planning and ISP development, with supervision provided to Direct Support Professionals.

Qualifications

• BA/BS Degree required in social work, psychology, human services or related field.
• One year of experience in the field of developmental disabilities.
• Supervisory experience preferred.

Responsibilities

• Actively participates in person centered planning and ISP development.
• Designs and implements support and teaching strategies to help consumers reach ISP outcomes.
• Supervises Direct Support Professionals and provides training on PCP/ISP implementation.
• Monitors progress and revises PCP/ISP outcomes based on data and feedback.
• Completes monthly outcome reports and submits to the Case Manager.
• Coordinates Medicaid Waiver guidelines and licensing standards with the interdisciplinary team.
