Job summary

Qualifications

• Bachelor's Degree in a field eligible for QDDP certification.
• Minimum of two (2) years of experience in a transferable field.
• Minimum of one (1) year of supervisory experience.

Responsibilities

• Lead the development, review, and ongoing coordination of person-centered service strategies to support goals, behaviors, and quality of life.
• Serve as primary coordinator among individuals served, families, partners, case management, healthcare providers, and staff to ensure effective communication and continuity of services.
• Support workforce planning by participating in talent acquisition, outreach, interviews, and onboarding recommendations.
• Oversee staff readiness by identifying training needs, coordinating certifications, and maintaining competencies.
• Facilitate planning meetings with interdisciplinary teams to evaluate progress and identify opportunities for improvement.
• Prepare and review service records, assessments, progress summaries, and regulatory reports for accuracy and compliance.
• Conduct quality assurance activities to evaluate service delivery and outcomes.
• Coordinate health and wellness oversight including preventive care, referrals, and follow-up services.
• Identify barriers to service delivery and implement corrective actions and resource coordination.
• Monitor compliance with policies, funding requirements, and regulatory standards while promoting person-centered practices.
• Maintain relationships with community agencies and healthcare providers to expand resources for individuals served.
