Help at Home in Brunswick, GA is seeking a Qualified Developmental Disability Professional (QDDP/DDP) to lead person-centered planning, ISPs, and staff supervision within our community-based team. Qualifications

• BA/BS degree in social work, psychology, human services or related field.
• At least one year of experience in developmental disabilities.
• Supervisory experience preferred.

Responsabilités

• Participate in person-centered planning and ISP development.
• Design and implement support strategies to help consumers achieve ISP outcomes.
• Provide supervision and training to Direct Support Professionals.
• Monitor PCP/ISP implementation and revise outcomes as needed.
• Coordinate with Case Manager and Interdisciplinary Team.
