Infusion for Health, a growing infusion center serving patients across multiple states, is hiring a Senior Provider Liaison Director in Brea, CA. Qualifications

• 5+ years in healthcare, preferably in provider relations and referral management.
• 2+ years of people leadership experience.
• Experience with infusion, specialty care or outpatient services.
• Familiarity with EMR, CRM and referral tracking systems.
• Ability to work cross-functionally with operations teams.
• Strong analytical and performance-driven mindset.
• Excellent relationship-building with providers and internal teams.
• Ability to lead in a fast-paced scaling environment.

Responsibilities

• Lead, coach, and develop a team of 10-15 Provider Liaisons.
• Set clear expectations, KPIs, and accountability around referral growth and patient journeys.
• Conduct regular 1:1s and performance reviews.
• Build a metric-driven, high-accountability culture.
• Oversee and optimize referral growth and patient conversion strategies across territories.
• Ensure strong relationships with referring providers and practices.
• Guide team on outreach cadence and problem solving.
• Partner on new market launches and service line expansion.
• Coordinate with Intake, Scheduling, Authorization, and Clinical teams.
• Identify and remove barriers to referrals (delays, documentation gaps, etc.).
• Build workflows to prevent referrals from falling through.
• Lead service recovery efforts.
• Monitor referral pipeline and CRM reporting; translate data into strategy.
