Job summary

Infusion for Health in Brea, California is seeking a Senior Provider Liaison Director to lead and scale a high-performing team of Provider Liaisons responsible for driving referral growth, patient retention, and provider relationships.

This role sits at the intersection of business development, inside sales, and patient operations, coordinating with Sales, Operations, and Clinical teams to ensure a robust referral pipeline and seamless service delivery across territories.

Qualifications

• 5+ years in healthcare, with provider relations and referral management.
• At least 2 years in people leadership and team coaching.
• Strong understanding of referral-based healthcare models (infusion, etc.).
• Experience in infusion, specialty pharmacy, or outpatient services.
• Familiarity with EMR, CRM platforms, and referral tracking systems.

Responsibilities

• Lead, coach, and develop a team of 10-15 Provider Liaisons.
• Set clear expectations, KPIs, and accountability around referral growth and patient journeys.
• Conduct regular 1:1s and performance reviews.
• Build a metric-driven, high-accountability culture.
• Oversee and optimize referral growth and patient conversion strategies.
• Maintain strong relationships with referring providers and practices.
• Guide strategic outreach cadence and problem solving.
• Partner with leadership to identify high-value referral sources.
• Support new market launches and service line expansion.
• Coordinate with Intake, Scheduling, Authorization, and Clinical ops.
• Identify barriers and remove delays in referrals.
• Build workflows to ensure no referral falls through the cracks.
• Lead service recovery efforts in escalated concerns.
• Track referral trends and diagnose leakage using CRM data.
