Job summary

Oak Street Health is seeking a Primary Care Physician to provide equitable value-based healthcare for local Medicare populations across neighborhood centers. You will work with a large care team to focus on quality care rather than volume, with a culture that rewards outcomes.

Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate.
• Board certification in Internal Medicine or Family Medicine (or eligible).
• Active, unrestricted medical license in state.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and monitor medications.
• Maintain Patient Electronic Medical Record data via EMR systems.
• Collaborate with leadership to meet health quality goals.
• Direct and support the center care team.
• Other duties as assigned.
