Job summary

Conviva Senior Primary Care, a Humana Inc. subsidiary, seeks a full-time Primary Care Physician for its Ormond Beach, FL center. The role emphasizes outpatient care, continuity of care, health maintenance, and disease prevention within a value-based framework.

Qualifications

• MD or DO, board certified or board eligible in Family Medicine or Internal Medicine; Geriatrics a plus.

Responsibilities

• Provide outpatient primary care with emphasis on continuity of care and preventive health.
• Maintain medical history and records; coordinate referrals to specialists as needed.
• Support a value-based care model and contribute to clinical team discussions.
