Job summary

Humana Inc. is seeking a Physician to provide outpatient care, focus on continuity of care, and participate in a value-based care model within Conviva Senior Primary Care in Middleburg, VA.

Qualifications

• Current and unrestricted medical license in state of practice
• Graduate of accredited MD or DO program
• Board Certification or Board Eligible in Family Medicine or Internal Medicine
• Active and unrestricted DEA license
• Excellent verbal and written communication skills
• Engage in integrated team-based care model
• Willingness to practice in value-based care setting
• Strong patient/customer service
• Familiarity with EMR systems
• TB screening participation as part of role

Responsibilities

• Evaluates and treats center patients in accordance with standards of care
• Follows level of medical care and quality for patients and monitors care using available data and chart reviews
• Assists in coordination of patient services including referrals and hospital coordination
• Acts as active medical expert within the care team
• Supports leadership in setting professional and cooperative practice tone
• Completes medical record documentation promptly with accuracy
• Complies with policies defined by Clinical Leadership
• Meets with leadership on quality of care and outcomes data
• Participates in growth opportunities for new or existing services
• Engages in local primary care on-call program as needed
• Maintains licensing and certifications
• Spends 100% of time clinically focused on direct patient care
