Job summary

ChenMed is seeking a Primary Care Physician (PCP) to join our physician-led, mission-driven team. You will provide comprehensive primary care to an assigned panel of patients, focusing on outcomes, cost-effectiveness, and compassionate, high-quality health care.

In this role, you will lead a care team, collaborate with hospitalists, and engage in risk and quality management programs. An MD or DO with applicable license is required, with board certification preferred and board eligibility

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar.
• Must obtain State Medical License for the state(s) of employment.
• Board certification preferred; Board eligibility required.
• Active DEA number required.
• BLS certification within first 90 days.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Take accountability for patient care and outcomes; seek consultations as needed.
• Collaborate with hospitalists during hospital stays.
• Assess, diagnose, treat, manage, educate and coordinate care.
• Lead care team including medical assistant, care facilitator, and care coordinator.
• Coordinate care for patients in hospital, SNF, LTC or homebound settings.
• Cover for other providers when needed; participate in recruiting.
• Supervise APPs and ensure adherence to guidelines.
