Job summary

ChenMed is seeking a Primary Care Physician (MD/DO) to join a physician-led, mission-driven primary care organization focused on delivering affordable VIP care to seniors. The PCP will manage a patient panel of up to 450, provide accountable, outcomes-driven care and lead a care team in a value-based model.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Board certification in Internal Medicine, Family Medicine or Geriatrics preferred; board eligibility required.
• Current DEA number for schedule II-V controlled substances.
• BLS certification required within first 90 days of employment.
• Fluency in English and ability to travel locally as needed.

Responsibilities

• Provide care for patients with acute and chronic illnesses, primarily older adults.
• Take accountability for patient care, outcomes, and consults when needed.
• Collaborate with hospitalists, case managers, and care teams to coordinate care.
• Lead and coordinate the office care team including medical assistant and care coordinators.
• Engage in risk and quality management programs and required meetings.
