Job summary

ChenMed is seeking a Primary Care Physician (PCP) in California to lead patient care for an older adult population within a high-quality, outcomes-focused, value-based model.

As a physician-led, mission-driven organization, ChenMed emphasizes accountability, health coaching, and teamwork. The PCP will work with center leadership to achieve quality outcomes and strong patient relationships.

Qualifications

• MD/DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Active State Medical License required.
• Board certification preferred; Board Eligibility required.
• Must have a current DEA number for schedule II-V.
• BLS certification within first 90 days of employment.

Responsibilities

• Provide care for patients with acute and chronic illnesses in older adults.
• Take accountability for patient care outcomes and coordinate with specialists when needed.
• Engage with hospitalists for patients admitted to hospital.
• Manage assessment, diagnosis, treatment, education and care coordination.
• Lead care teams including medical assistant, care facilitator and care coordinator.
• Collaborate with transitional care and case management for non-office visits.
• Cover for other providers and participate in recruiting.
