Job summary

ChenMed LLC is seeking a Primary Care Physician to lead comprehensive care for seniors in a value-based, physician-led system. The PCP will manage a panel of up to 450 patients, act as a health coach, and drive actionable care plans to improve outcomes.

You will collaborate with center leadership, managers and medical directors to ensure guideline adherence, participate in risk and quality programs, and recruit patients and providers as needed. Flexible to travel for training and recruiting.

Qualifications

• MD or DO degree and state license
• Board certification preferred
• Ability to travel locally and nationally as required

Responsibilities

• Provide care for older adults with acute and chronic illnesses
• Own patient outcomes and coordinate care including consulting specialists as needed
• Collaborate with hospitalists, case managers, and care teams
• Lead care teams (MA, care facilitator, care coordinator) and coordinate for office-visit and in-hospital patients
• Recruit patients and providers to support center growth
• Supervise APPs and collaborate within practice agreements
• Perform other duties as assigned by management
