Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective primary care to adults on Medicare in our outpatient clinics. You will practice independently within a collaborative care team including physicians, NPs/PAs, medical assistants, a scribe, RNs and care managers.

You will lead wellness visits, health risk assessments, and care coordination to improve population health and patient outcomes while building lasting patient relationships and ensuring high-quality documentation.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• National certification as an NP in one of the specified specialties.
• Active, non-probationary NP license in the applicable state.
• Active DEA license and US work authorization.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Deliver longitudinal care and build trusting patient relationships.
• Work within a collaborative care team including physician, NP/PA, MAs, scribe, RNs and care managers.
• Contribute to care coordination, population health and breast cancer screening reminders.
• Educate patients on health conditions, care plans and treatments.
• Participate in promotional activities and, when needed, conduct home visits.
• Report to Center Medical Director and coordinate with operations leadership.
