Job summary
Oak Street Health seeks a Nurse Practitioner to provide effective, value-based primary care to adults on Medicare in outpatient clinics. NPs work autonomously with a care team to build lasting patient relationships and empower patients in their care plans.

Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- Active, non-probationary state NP license and DEA license.
- US work authorization is required.
- Experience in primary care is preferred.
Responsibilities
- Provide effective and equitable value-based primary care to adults on Medicare.
- See patients independently and collaborate with physicians as needed.
- Deliver longitudinal care and build patient relationships.
- Perform annual wellness visits and health risk assessments.
- Coordinate care with the care team and external specialists.
- Educate patients on health conditions and care plans.
