Job summary
Conviva Senior Primary Care in Ormond Beach, FL is hiring a full-time Primary Care NP or PA to deliver comprehensive geriatric care on-site within a collaborative team. You will coordinate referrals, hospital/SNF coordination, DME and home health services, and document care with support from quality coders.
The role requires Florida NP/PA licensure with board certification, and at least one year in value-based or Medicare Advantage settings.
Qualifications
- Master's degree in nursing with board certification or PA program with board certification.
- Active, unrestricted Florida NP or PA license.
- At least one year of value-based care or Medicare Advantage experience.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain.
- Familiarity with HEDIS indicators and Medicare guidelines.
Responsibilities
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with the care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Document care with support from quality-based coders.
- Participate in on-call rotation and support growth plans at your center.
- Work on-site.
