CenterWell Senior Primary Care is seeking a full-time NP or PA to join our West Orange clinic in Orlando, FL, with flexible coverage across the Orlando market. You will work within a multidisciplinary team to deliver high-quality, value-based care to seniors.

The role requires a master’s degree in nursing or a board-certified PA program, and an active Florida license. Relocation and a sign-on bonus may be available as part of our comprehensive benefits package.

Qualifications

• Master’s degree in nursing or PA program with board certification.
• Active, unrestricted NP or PA license in Florida.
• Experience in value-based care or Medicare Advantage settings.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from coders.
• Participate in on-call rotation and support center growth.
