Job summary

CenterWell Senior Primary Care is seeking a full-time NP or PA to join a collaborative, multidisciplinary team in Orlando, FL. You’ll work from our West Orange clinic with flexible coverage across the Orlando market, delivering value-based, senior-focused primary care.

You’ll report to the Associate Medical Director and enjoy guaranteed base salary plus a bi-annual bonus, comprehensive benefits, CME support, and relocation/sign-on incentives.

Qualifications

• Master's degree in nursing or PA program with board certification.
• Active, unrestricted NP or PA license in Florida.
• Minimum one year experience in value-based care, primary care or Medicare Advantage settings.
• Minimum one year experience as an APP.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately and efficiently with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Demonstrate passion for value-based care.
