Job summary

Conviva Senior Primary Care, part of Humana’s Primary Care Organization, is hiring a full-time Nurse Practitioner or Physician Assistant to join our Edgewater, FL clinic. You’ll deliver comprehensive, seniors-focused care within a collaborative team that includes physicians, pharmacists, care coaches, and behavioral health specialists.

You’ll report to the Associate Medical Director and work on-site at the Edgewater clinic, with a base salary plus bonuses, relocation/sign-on options, and a full

Qualifications

• Master’s degree in nursing with board certification.
• Active, unrestricted NP, or PA license in Florida.
• Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at your center.
• Demonstrate experience with value-based care.
• Work on-site.
