Job summary

Conviva Senior Primary Care in Edgewater, FL is seeking a full-time Primary Care NP or PA to join a collaborative team delivering comprehensive, value-based care to seniors. You will report to the Associate Medical Director and work on-site at our Edgewater clinic, coordinating with physicians, pharmacists, care coaches and other specialists.

Qualifications

• Master's degree in nursing with board certification.
• Active, unrestricted NP, or PA license in the state of Florida.
• Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
• Familiarity with HEDIS quality indicators and Medicare guidelines.

As a Nurse Practitioner (NP) or a Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

What We're Looking For

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
