Job summary

Country Hills Post Acute seeks a Staff Physical Therapist to evaluate and treat residents, coordinate with families and physicians, and maintain timely documentation. The role includes supervising PTAs and providing staff in-services to support rehabilitation goals.

Responsibilities also cover discharge planning, equipment orders, infection control, and activity to enhance mobility and safety for residents. Licensure and CE requirements apply.

Qualifications

• Licensed as a Physical Therapist in the state and maintain licensure in good standing.
• Continued state CE requirements must be met.
• CPR certification, if required by licensure.
• Proficient with Net Health and Point Click Care computer systems.
• Ability to read technical procedures and policy manuals.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise Physical Therapy Assistants and students in resident care.
• Communicate with physicians, nursing staff, and families regarding progress and discharge plans.
• Provide in-services and training for facility staff; contribute to QA and program development.
• Maintain timely, compliant documentation per facility and payer requirements.
• Educate residents and families on home exercise, safety, and fall prevention.
• Assist with ordering of necessary durable medical equipment.
