Job summary

Beachwood Post Acute & Rehab is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and coordinate with families and physicians. You will lead rehabilitation efforts and ensure plans align with physician orders to restore function.

The role includes supervising PT assistants, providing in-services, and maintaining accurate documentation while upholding infection control and safety standards during patient care.

Qualifications

• Bachelor’s degree in Physical Therapy; Masters/Doctorate preferred.
• Proficient in Net Health and Point Click Care.
• Language skills to read technical procedures.
• Ability to interpret policy and procedure manuals.
• Ability to present information clearly to managers, coworkers and families.
• Mathematical skills to apply fractions, percentages and ratios.
• Reasoning ability to solve practical problems.
• Ability to follow written, oral, diagram or schedule instructions.

Responsibilities

• Evaluate residents to determine PT needs, functional limitations, and rehab potential per physician orders.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Assist nursing with training Restorative Aides.
• Supervise PT assistants in resident care following state practice act.
• Communicate progress and discharge plans with physicians, nursing staff, families.
• Participate in Resident Care conferences and Rehabilitation meetings as needed.
• Provide in-services on training programs for facility staff as needed.
• Complete timely, accurate documentation per requirements.
• Maintain safe treatment environment with infection control and equipment maintenance.
• Educate residents and families on goals, home programs, and fall prevention.
• Recommend durable medical equipment to facilitate independence (splints, walkers, etc.).
