Qualifications

• Bachelor's degree in Physical Therapy.

Responsibilities

• Evaluate residents to determine physical therapy needs and rehab potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PT Assistants in resident care and activities.
• Communicate with physicians, nursing staff, and families about progress and discharge planning.
• Participate in Resident Care conferences and Rehabilitation meetings.
• Provide in-services on training programs for staff as needed.
• Complete timely, accurate documentation in line with requirements.
• Maintain infection control and safety protocols in therapy areas.
• Educate residents and families on home exercise, safety, and fall prevention.
• Recommend durable medical equipment to facilitate independence.
