Job summary

Alta-Heights-Post-Acute seeks a Staff Physical Therapist to evaluate residents, develop personalized treatment plans, and coordinate with physicians, families, and staff. You will supervise PT assistants, train nursing staff, and participate in care conferences to optimize rehabilitation outcomes.

The role emphasizes documentation compliance, patient education, and safe handling practices while enabling residents to regain mobility and independence in a supportive post-acute setting.

Qualifications

• Licensed Physical Therapist in the state with active license.
• Continuing education requirements must be met per state practices.
• CPR certification, if required by licensure.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Assist nursing with training Restorative Aides and supervise PTAs.
• Communicate progress, goals, and discharge plans with physicians and families.
• Participate in Resident Care conferences, Utilization Review, and Rehab meetings.
• Provide staff in-services and ensure timely, compliant documentation.
• Maintain safe treatment environment and infection control standards.
• Educate residents and families on goals, home programs, and fall prevention.
• Recommend and facilitate ordering of durable medical equipment.
