Job summary

The Staff Physical Therapist at PACS evaluates residents, plans rehabilitation, and educates families and staff. You will implement individualized treatment programs to improve mobility and independence, while supervising PT assistants and coordinating with physicians.

Documentation must be accurate and compliant, and infection control practices must be followed. Licensed PT in the state is required, with continuing education and CPR as applicable.

Qualifications

• Bachelor’s degree in Physical Therapy required or equivalent.
• Masters or Doctorate in Physical Therapy preferred.
• Proficient with medical records software and documentation.
• Licensed Physical Therapist in the state; CPR may be required.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility.
• Supervise PT Assistants and support staff in care activities.
• Communicate with physicians, nursing staff, residents, and families.
• Complete timely, accurate documentation per policies and regulations.
• Maintain safe treatment environments and follow infection control policies.
