Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat residents, communicate with families and physicians, and maintain thorough documentation. You will help implement treatment plans to improve mobility and independence and supervise PT assistants with attention to safety and compliance.

Responsibilities include training facility staff, coordinating with the interdisciplinary team, and ensuring accurate records while upholding infection control standards.

Qualifications

• Licensed Physical Therapist with state license
• Strong ability to assess resident mobility and rehabilitation potential
• Proficient in documenting care plans and progress notes

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and ensure compliant documentation.
• Communicate with physicians, nursing staff, residents and families about progress.
• Educate residents and families on home exercise programs and safety.
