Job summary

PACS is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and improve mobility. You will supervise PT assistants and coordinate with physicians and families to support discharge planning and follow-up care.

The role requires licensure in the state and proficiency with Net Health and PointClickCare. You will contribute to training and quality initiatives within the rehabilitation program.

Qualifications

• Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Licensed Physical Therapist in the state; maintain license in good standing.
• Proficient with Net Health and PointClickCare data entry.
• Ability to read technical procedures.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants, aides, and students.
• Communicate with physicians, nursing staff, families; participate in care conferences.
• Complete compliant documentation; ensure infection control and safety.
