Qualifications

• Outpatient patient outreach and telephonic engagement experience.
• Ability to build rapport and assist patients remotely.
• Experience obtaining medication histories and addressing adherence or safety concerns.
• MTM or Medication Historian experience preferred.

Responsibilities

• Identify eligible patients for Population Health programs.
• Conduct outbound patient outreach by phone.
• Collaborate with patients, providers, and care teams on medication initiatives.
• Assist with medication reconciliation and histories.
• Coordinate interventions to improve quality metrics and outcomes.
• Document interactions and maintain records per standards.
• Perform patient outreach and chart reviews as needed.
• Escalate issues to the pharmacist or Population Health team.
