Mount Carmel, part of Trinity Health, seeks a Pharmacy Technician on the Population Health team to support pharmacy initiatives and payer quality programs for Health Partners members. This hybrid role requires collaborating with patients, providers, and care teams to improve medication adherence and health outcomes.

You will conduct outbound patient outreach by phone, reconcile medications, collect histories, document interactions in the EHR, and assist with MTM tasks.

Qualifikationen

• Experience in outpatient patient outreach and telephonic engagement.
• Strong communication skills with the ability to build rapport remotely.
• Active Certified Pharmacy Technician registration with OSBP is required.
• Experience obtaining medication histories and addressing adherence or safety concerns is preferred.

Aufgaben

• Identify eligible patients for Population Health programs and interventions.
• Conduct outbound outreach by phone and engage patients to improve adherence.
• Collaborate with providers and care teams to support medication initiatives.
• Perform medication histories and reconciliation as needed.
• Coordinate interventions to improve quality metrics and health outcomes.
• Document interactions and maintain accurate records in the EHR.
• Gathers data and generates reports to improve medication-related quality metrics.
• Participate in training and orientation of new staff as needed.
