Job summary

Providence Centralia Hospital seeks a Senior Clinical Pharmacist to collaborate with physicians and staff to deliver coordinated, high-quality patient care. You will participate in patient-focused, whole-person care within a culture of understanding and mutual respect.

Qualifications

Bachelor's Degree in Pharmacy from an accredited School of Pharmacy or PharmD from an accredited School of Pharmacy.
Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
Washington Pharmacist License upon hire.
Board of Pharmacy Specialties certification within 2 years of hire.
Board of Pharmacy Preceptor License when eligible and applicable.
3 years Pharmacy practice experience with at least 2 years of related experience in lieu of PGY-1.

Responsibilities

Provide collaborative practice services in collaboration with the healthcare team.
Utilize registries for chronic diseases and reports/tools to identify target populations and enhance therapeutic outcomes through education and drug therapy optimization.
