Qualifications

• PGY-1 Pharmacy Residency or equivalent education/experience.
• Washington Pharmacist License upon hire.
• Within 2 years of hire: Board Certification from BPS.
• Board of Pharmacy Preceptor License upon hire where applicable.
• 3 years of Pharmacy practice with at least 2 years related experience if PGY-1 not completed.

Responsibilities

• Provide collaborative practice services in collaboration with the healthcare team.
• May use registries for chronic diseases and reports/tools to target populations and improve outcomes.
