Job summary

Providence Centralia Hospital seeks a Senior Clinical Pharmacist to collaborate with physicians and staff for coordinated, high quality patient care. The role focuses on patient care and population management within a team-based setting at PRCS Lacey-Lacey.

Responsibilities include collaborative practice services and utilization of registries to improve disease management and drug therapy optimization, with comprehensive benefits and on-site work.

Qualifications

• Bachelor's Degree in Pharmacy from an accredited School of Pharmacy or
• PharmD in Pharmacy from an accredited School of Pharmacy.
• Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
• Washington Pharmacist License upon hire.
• Within 2 years of hire: Certification from Board of Pharmacy Specialties.
• Board of Pharmacy Preceptor License when eligible and where applicable.
• 3 years Pharmacy practice experience with at least 2 years of related experience in lieu of PGY-1.

Responsibilities

• Provide collaborative practice services in collaboration with the healthcare team.
• May utilize registries for chronic diseases and reports/tools to identify target populations and optimize therapy.
